This certifies that, I completed this application and that all entries on it and information in it are true and complete to the best of my knowledge. I authorize GUILDFORD CAB (1993) LTD. to make such investigations and inquiries of my personal, employment, medical history and other related matters as may be necessary in arriving at decision. I authorize GUILDFORD CAB (1993) LTD. to make such investigations and inquiries of my personal, criminal search, driving abstracts, Claim History, PSP reports (Pre-Employment Screening Program), drug results from previous employers or their consortium or any other related matters as may be necessary in arriving at decision. If hired or contracted, this authorization shall remain on file and shall service as ongoing authorization to re-check or report as deemed necessary at any time throughout contract period or after such period. Furthermore, I understand that the GUILDFORD CAB (1993) LTD. may keep any information on file including Driving performance as related to my contract period and make it available to any second party only with my verbal or written consent. I hereby release employers, schools, health care providers and other persons from all liability in responding to inquiries and releasing information in connection with my application. I understand that as an applicant for a position with GUILDFORD CAB (1993) LTD., I may be asked to demonstrate that I am capable of performing tasks that are pertinent to the Taxi Driving. In the event of contract, I understand that false or misleading information given in my application or interviews, or information omitted from my application or interviews, may result in an immediate termination. It is agreed and understood that this application for Driving Privileges in no way obligates the GUILDFORD CAB (1993) LTD to hire the applicant. I understand also that I am required to abide by all rules and regulations of the Company, as permitted by law. For purposes of gathering this information, I agree to supply the information which may be required by law enforcement agencies and other entities for positive identification purposes when checking records.
